Please provide the title and a short (no more than 50 words) abstract for the research project for which you intend to use the Data Enclave.
Provide a short description of your proposed research project including research questions, methodology, and categories of variables to be used. Also substantiate why public-use files (if available) are not adequate for the conduct of your research (note: attach additional sheets if necessary).
Describe the proposed analysis and expected analysis results, including a list of study variables contained in the output you plan to export.
Describe how you intend to use the results of the research, including plans for public dissemination.
If you plan to merge your data with the restricted data, provide detailed description of your data, including documentation, file layout, number of records, and records per case.
The following statistical software resides in the Data Enclave for approved researchers: SAS and Stata. Please indicate whether you require any additional software.
List any special needs in terms of your use of the Data Enclave.
Describe the funding source (if any) for this research effort.
Please attach a copy of your resume/vitae.
Attach the IRB approval from your sponsoring institution for this project.
Attach two signed copies of the Confidentiality Agreement for internal use.
This Agreement will go into effect upon approval of the Agreement by NORC and will remain in effect until the completion of the research project, as noted in the Data Use Application. This Agreement shall be construed in accordance with and governed by federal law, without giving effect to the conflict of laws provisions thereof. The parties that the individual, or class of individuals identified above are designated custodians of the restricted data on behalf of the recipient organization and will be personally responsible for the observance of all conditions of use and for establishment and maintenance of physical and data safeguards, as required by applicable state and federal laws, to prevent unauthorized access, disclosure, or use.
